Two things fix gender. A role word does it directly — mother, father, sister, daughter. A role a person holds towards someone else does it indirectly, which is how Q is fixed as male here, by fathering R.
Decode the chain into short sentences first, then draw two generations with the men and women marked. The relationship asked for usually falls out of the drawing rather than the sentences.
The question asks how P is related to S, not how S is related to P. The same family fact reads as mother-in-law from one side and as husband's mother from the other, and the options here use the second phrasing.
